Detecting adenovirus shouldn't delay immunomodulatory treatment when a child meets Kawasaki disease (KD) criteria. This new editorial in #CJCPC discusses an article by McCrindle et al. showing concomitant adenovirus yields a milder phenotype with zero coronary artery involvement—reinforcing prompt IVIG over diagnostic hesitation. New retrospective analysis in #CJC: Evaluating long-term implantable loop recorder (#ILR / #ICM) safety, diagnostic yield, and explantation economics ($n = 772$, 2011–2025). Key findings:

• Extended Dwell Time: 46% retained devices beyond the 3.25-year recommended lifespan (mean dwell time 5.5 $\pm$ 2.2 yrs in this subgroup).

• Safety Profile: Complication rate peaked within the first 180 days (4.13 per 100 device-years) and did not significantly increase beyond the recommended replacement window (IRR 1.36, $p = 0.24$).

• Diagnostic Yield: True symptom-rhythm correlation established in 38.4% of patients.

• Health Economics: In-hospital explantation cost ≈$490 CAD vs ≈$70 CAD in a simulated community clinic setting—an 85.7% cost reduction.

Extending ILR dwell time is clinically safe, supporting deferred explantation strategies and transitioning device removals to community outpatient models to alleviate acute hospital capacity constraints. Read here ⬇️

This new #CJCO systematic review/meta-analysis examines high-resolution OCT characterization of coronary intravascular lithotripsy (#IVL) outcomes across 30 studies and 3,030 patients.

Key mechanical & procedural findings:
• Calcium Fracture Rate: 68% (95% CI 56–79%), with multiple fractures in 34%
• Final Minimum Stent Area (MSA): 6.45 mm² (95% CI 6.07–6.82)
• Stent Expansion: 85.9% (95% CI 82.3–89.5%)
• Acute Lumen Gain: 3.50 mm² (95% CI 3.14–3.87)
• Procedural Success: 95% (95% CI 93–98%)
• In-Hospital MACE: 2% (95% CI 1–3%)

Intracoronary imaging confirms that acoustic shockwaves reliably disrupt heavy vascular calcium to deliver optimal stent expansion with low acute complication rates. Read here ⬇️

New in #CJCPC: The novel ACHD PRO metric correlates strongly with physician-assessed NYHA functional class in adults with congenital heart disease—with an 11-point change in summary score signaling a 1-category shift in NYHA class. A validated tool for capturing functional decline early. Read here ⬇️
