More dose was not better in definitive ESCC. Three-year PFS: 45% with 50.4 Gy vs 26% with 60 Gy (phase 2, n=97), but the difference vanished after covariate adjustment. Single-center trial using S-1/nedaplatin. No support for escalation.

MASTERPLAN: Adding 40 Gy/5 fx SBRT to chemotherapy cut 12-month locoregional recurrence in high-risk localized pancreatic cancer from 52% to 20% (phase 2, n=74). The trial closed early at 74 of a planned 120. Enough to warrant phase 3?

Seven prostate RT fractions held up at 10 years. HYPO-RT-PC: FFS was 72% with 42.7 Gy/7 vs 65% with 78 Gy/39 (phase 3, n=1,180), meeting noninferiority; GU/GI toxicity was similar. Caveat: 89% had intermediate-risk disease and no ADT.

For painful bone mets, 8 Gy x2 did not reduce 6-month retreatment vs 8 Gy x1 (10.3% vs 11.5%; phase 2, n=102). Pain response and QOL were similar. Small trial, but the extra visit showed no signal of benefit. Is 8 Gy x1 your default?

Randomized H&N RT study (n=83): AI-assisted GTV contouring did not reduce peer-review edits (median APL difference: -0.02 for primary, +0.06 for nodal GTVs) or save time. Single-center and exploratory. Are we validating targets, not just OARs?
