Adjunctive targeted disruption of the Purkinje network improved outcomes after VT ablation in patients who also had evidence of VF. It is critical to review every episode of ICD therapy and plan accordingly.

A #JACCCEP study showed that in pts. w/ prior spontaneous polymorphic VT/VF, adjunctive Purkinje de-networking during VT #epAblation was associated with a substantial reduction in recurrent ventricular arrhythmias, w/ a modest increase in risk of LBBB.

When we consider everything Michel Haissaguerre has already done for our field it is beyond inspirational to see how he continues to share his knowledge with us. The MEJ award is well deserved.

Fascinating to consider the acute and longer term effects of PFA on fascicular tissue. I suspect that there will be a lack of durability for Purkinje tissue as it has a much lower metabolic demand compared to myocytes. Might need to be prepared for temporary pacing overnight.

Can we use PFA selectively to treat the myocardium and spare the conduction system? #ablateVT #Epeeps
