This concept has 2 extensions IRL:

1. Consults carry w them a threat of advice in the wrong direction and might make me not want to call or have to write a looong note defending my care.

2.

if your ID department has both antibiotic-maximalists and antibiotic-minamilists, it needs to be broken into TWO different services so you can ask for what you need:

ABX-minamilist service
- consult them when someone with nonpurulent cellulitis is put on three antibiotics
- everything is treated wih either PO amoxacillin or PO doxy
- keep the patient’s neutrophils busy

ABX-maximalist service
- consult them when patients are getting worse and you’re scared
- everyone gets $10,000 worth of PCRs & fungal antigens
- they agree on using the CDC’s reserve of 10th-generation cephalosporin alongside meropenem-avibactam with a side of IVIG and isavuconazole

Even in ED before fluid:

Dizzy/lightheaded/presyncope- “Well i didn’t drink anything this morning, guess I’m a bit dehydrated.”

Bruh. You haven’t had bkfast since 1985 and today is when it all caught up w you? Sell it somewhere else.

Residents? Stop repeating their lies.

This is one of the funniest things to me. I will often ask someone who tells me that they’re dry. How is it that they got that way and then there’s usually a little bit of silence, a mumble about insensible losses, a limited PO intake, despite the 3 L that have been given in the last 48 hours, lol. The reality is that unless you were getting aggressive diuretic therapy or you have severe G.I. losses, your so-called intravascular dry patient likely has a main systemic filling pressure problem and you don’t wanna miss the septic source because you thought they were dry.
