New edition of @JACCJournals #JACCcasereports -- one of the papers I reviewed from @UCCardsFellows @UofC @jbspadoni on percutaneous removal of fibroelastoma (a great 'How we did it'... TEE guided, snare assisted angiovac of the PFE).

@vandanMD , myself and @ryankaple editorial link below
Summary:
1.) CEPD mandated (for now, we have to assume embolization rate >1%.. we don't have any large case-series)
2.) Risk of fibroelastoma causing CVA vs procedure carefully weighed
3.) If you are going to do it-> institution should be comfortable with left sided angiovac procedures and electrosurgery

Nail the ostium better than LM crossover? Hold your horses!
jscai.org/article/S2772-…
Retrospective study defining ostial lesions by angiography (within 5 mm), not IVUS. LM crossover patients had more calcium (38.6% vs 23.3%) and required more lesion preparation.
Was ostial stenting chosen because a safe landing zone existed, or crossover because disease truly involved the ostium and was more severe? Selection bias matters!
Bottom line: IVUS confirms a safe landing zone, and clear ostial visualization on angiogram ->Ostial stent. True ostial disease or poor visualization ->LM crossover.
