Despite multiple PFA applications, unable to achieve anterior block for mitral flutter. Epicardial voltage with peak frequency emphasis highlighted RF target. Lateral splits seen after first TactiFlex application.

Patient Hx

3 yrs post Cryo PVI & CTI.

3 months post PFA of posterior wall.

Presented in 340 ms tachycardia. #HDGrid showed scarring with slow conduction through left atrial roof and floor.

Termination at isthmus, confirmed with entrainment, with #TactiFlex.

Great heart scan on ICE, always fun to see a false tendon!

Interesting variance in prevalence when researching journals, many
undiscovered due to imaging technique, TEE, TTE, versus ICE.
