This past week I joined @InsideEdition to talk about how multiple sclerosis (#MS) is diagnosed, and how far #MRI has come in helping patients reach an early and accurate diagnosis. Although time constraints for a short segment mean we can't include most of our conversation, this remains an important discussion.

In most patients, MS has a distinctive appearance on brain MRI. The appearance, distribution, and location of lesions allow #neuroradiologists to recognize the disease with confidence, and imaging is central to establishing the diagnosis.

Some presentations are less clear-cut, for example when there are only a few lesions or they are in less typical locations. These are the cases where two imaging markers add valuable information.

The central vein sign (CVS) reflects the fact that MS lesions form around small veins, whereas the microvascular ischemic changes we commonly see with age and vascular risk factors center on arterioles. Paramagnetic rim lesions (PRL) show a rim of iron at the lesion edge, a marker of chronic active inflammation. Both are highly specific for MS, and both are now incorporated into the 2024 revisions of the McDonald diagnostic criteria.

Detecting these signs depends on susceptibility-sensitive sequences acquired with an optimized protocol.

At @RadNetImaging , we optimize our MS protocols using the latest technology. Our referring neurologists also typically request the #AI tool of quantitative MRI (#QMRI), which measures new or enlarging lesions and brain volume over time with greater precision.

The outlook for patients is encouraging, particularly given the numerous disease-modifying therapies now available, which significantly reduce long-term disability. MS diagnosis today rests on clear criteria, close collaboration between neurologists and neuroradiologists, and imaging tools that continue to become more precise. Anyone with questions about their own diagnosis is best served by a conversation with their neurologist.

Last weekend at @theASFNR, my son, @AnderssonH27121, and I were on the same program for the first time. Of all the meetings I have been honored to speak at over the years, this one was the most meaningful because of his presence.

Hayden, a pre-med junior at @UCSanDiego, is an EMT, a NASA intern, a medical scribe in a neurology practice, and President of the UCSD Brazilian Jiu-Jitsu Club.

As lead author, he presented his first abstract, “Quantitative MRI After Long-Duration Spaceflight,” coauthored with his outstanding mentor, @BrainsOnMars, former Deputy Chief Scientist of the International Space Station National Laboratory, and me. Comparing pre- and post-flight MRI in 41 astronauts, the study measured quantitative biomarkers, and found measurable structural changes after long-duration missions.

I had the privilege of speaking on “#Precision #Neuroradiology: #AI Powered Objectivity and Insights.” I also spoke at the @Bayer sponsored Lunch & Learn on the “Era of Ultra-High Relaxivity: Contemporary GBCAs in Clinical Practice,” hosted by @Applied_Rad.

The social events were wonderful too, and playing poker with Hayden at the After Dark Party was a highlight!

Congratulations to President-Elect Dr. Vince Kumar for organizing such a wonderful meeting, and thank you to President Dr. @CoolAsANeuroRad for his leadership. A huge congratulations as well to founding #ASFNR President Dr. @AndreiHolodny, the inaugural Gold Medal recipient. So well deserved!

From a founding president receiving the society's first Gold Medal to a student presenting his very first abstract, this meeting celebrated our field at every stage of a career. I am already looking forward to next year.

I am truly humbled to be selected as a 2026 AuntMinnie.com "Best of Radiology Semifinalist" in two categories: "Most Effective Radiology Educator" and "Image of the Year."

Teaching has always been the most rewarding part of my career, and being recognized alongside so many educators I admire and have learned from is an honor in itself.

Congratulations to all of my outstanding peers named this year. Your research, teaching, and leadership continue to move our specialty forward, and it is a privilege to be on this list with you.

Congratulations as well to the many vendors recognized for innovations that are improving patient care, with a special shout-out to the following products:

• Reporting Pro, @DeepHealth
• NeuroQuant PET, @Cortechs_ai
• SubtleHD (PET), @SubtleMedical
• AiMIFY, @Bracco_Imaging/@SubtleMedical
• Viz Pulmonary Suite, @viz_ai

Thank you, #AuntMinnie, for championing excellence in radiology. I am deeply grateful.

The nominated images pair a positive #amyloid PET with #AI-based quantitative MRI (#QMRI) in a patient with #ARIA.
